A colour coded scale ranging in severity from 0-4. The overall report severity is based on comments, and not just individual test results.
Information about the account, component, sample, filter, lubricant, and additional (optional) information which can include the time at which the sample was taken of the initials of the person taking the sample.
An analysis of the test reults, including maintenance recommendations & feedback from our data analysis team.

A This portion must be logged online and must be completed each time you submit a sample. More detail provided = A more accurate and detailed analysis of your component
B Component registration only needs to be completed once, usually the first time you submit a sample for each component.
C Attach one barcode to the sample jar and retain the other for your records. If you logged your sample online, the barcode will contain all required sample information needed for processing.
D Place the Edmonton, AB address label on the (black) mailer bottle and send your package off to our lab. Remember to always use a trackable shipping service when sending us samples!
